The flow of oil and toxic gases are bringing along sand and gravel which causes the flow to create an effect of sanding on the rest of the wellhead which now somehow restricts the flow, but also the hole drilling.
As the wellhead becomes increasingly eroded the passage increases allowing a greater flow of oil. Even if a device was placed over the head of the existing well, that does not stop and would cap the flow, because what is left of the wellhead does not contain the pressure of the flow.
